What is blepharoplasty?

The passage of time, genetic predisposition, or an injury or illness can cause eyelid drooping, which will manifest as the upper eyelid lowering more than it should (ptosis) or an excess of skin (dermatocalasis). The ultimate cause may be muscle weakness, damage to the nerves that control the eyelid, or loss of skin elasticity.

Blepharoplasty is a plastic surgery whose objective is the elimination of excess skin from the eyelids, both upper and lower, as well as the extraction of underlying fatty tissue (usually called bags). In the lower eyelid, it also improves dark circles. Blepharoplasty is designed individually, according to the needs and particularities of the patient.

Is blepharoplasty the right plastic surgery for me?

Treatment

Blepharoplasty improves skin laxity in the area, as well as eliminates bags, and can help improve vision in people with excess tissue in the upper eyelid that falls on the pupil, making vision difficult. It does not eliminate “crow’s feet” or other wrinkles, dark “circles”, or lift drooping eyebrows. We treat wrinkles with laser, peels, or Vistabel neurotoxin. Eyelid surgery can be performed in isolation, only on the lower or upper eyelids, or on both. In addition, it can be combined with other surgical procedures on the eyes, face, eyebrows, or nose.

Upper eyelid surgery

To correct the upper eyelids, the plastic surgeon draws a spindle of skin that corresponds to the excess amount, to later remove it through a small incision, with or without part of the orbicularis muscle, depending on the characteristics of the eyelid and what we want to achieve. After removing the skin, we proceed to extract the excess fat from the bags, mainly the internal one, which is closer to the nose. The scar is right in the natural crease of the eyelid, so it is not visible. Keep in mind that drooping eyebrows can be improved through a frontal or partial forehead lift or with direct excision of skin just above the eyebrow. We can also treat the muscles of the glabella permanently (so as not to have to depend on botulinum toxin) and even lift the tail of the eyebrow from here.

Lower eyelid surgery

For lower eyelid surgery, there are several techniques (classic, transconjunctival, Loeb and canthopexy) that will be assessed during the first consultation, according to the characteristics of the patient’s eyelids, their face and also their expectations.

In classic blepharoplasty, access is gained through an incision in the skin just below the lash line (subciliary) and excess skin, muscle and bags are treated. It is indicated in cases with excess skin or many wrinkles. You have to be very conservative to avoid the so-called round eye, that is, that the lower eyelid retracts and the sclera (the white of the eye) is seen more.

In transconjunctival blepharoplasty, access is gained from the inside, through the conjunctiva, and is used mainly to treat the bags, which are usually used to fill the tear trough (Loeb Technique). They are no longer extracted to avoid the sunken eye.

Canthopexy is the correction of the external canthus of the eyelid. Sometimes it has to be added to classic blepharoplasty to prevent the appearance of the round eye (due to retraction of the lower eyelid when scarring) and sometimes as an indication only to almond the shape of the eyelids more

Postoperative

Blepharoplasty can be performed with local anesthesia, local anesthesia plus sedation, or general anesthesia. The hematoma resulting from the intervention is greatly reduced if, after cosmetic surgery, ice is applied to the eyes for a couple of hours or a controlled cold system called hilotherapy, which is much more effective than cold with ice or patches. After four or five days, the stitches are removed and draining massages are started to reduce edema and give elasticity to the skin.

Complementary surgeries and aesthetic treatments

In order to optimize the surgical procedure, it is often combined with a facelift and treatment with neurotoxin (treatment with selective chemical myomodulation), or laser in order to improve the expression lines of the eyes, the wrinkles in the glabella and reposition drooping eyebrows.
