The traditional surgical technique to treat tubular breasts is to reshape the mammary gland. This can be achieved by unfolding the gland or opening it internally (“scoring”). The implant gives volume and projection in addition to contributing to the expansion of the gland. Anatomical implants work much better than round implants because they expand and distend the lower pole of the breast, which is usually the most constricted.
Currently there is another technique that is changing the way in which we solve the problem of breast tuberosity and that consists in the use of adipose tissue grafts obtained by liposuction of the flanks, abdomen and/or hips.
The most important thing is to change the shape of the gland, increasing the base of the gland, eliminating the sagging and expanding the skin. If the plastic surgeon does not do this and only places a breast implant, we usually get a double submammary fold or a double contour.
Usually two sessions are necessary and it does not achieve a large volume, but it does change the shape without the need for complex plastic surgery of the gland or implants. Another option, when you want more volume and increase the bra size two cups or more, is to combine the prosthesis and fat grafting, so that the fat is grafted throughout the lower and inner pole opening the fibrous ring that constricts the breast, freeing the groove and widening the base.
In mild cases of tuberous breasts (Type I) or breasts with normal shape and tuberous areola can be augmented by axillary route and combined with fat grafting to have a better shape and avoid scarring of the areola. There have been cases that the graft itself has normalized the shape of the areola without the need to make the scar around it.
However, if the areola is very protruded or large, it is necessary to remove skin around it and break the constriction around the areola. A white scar is left around it but it can be micropigmented after a year.
Tuberous breast (also called constricted breast, tubular breast or conical breast) refers to an abnormality in the development of the breast.
The breast is a modified gland of the skin that at a certain moment in a woman’s development and due to the action of hormones expands and grows. When this growth is not uniform, a tubular breast or mammary tuberosity is produced.
There are many different forms of mammary tuberosity and the severity of the deformity varies greatly. The mildest is when the areola protrudes more than usual due to excessive stiffness of the skin overlying the mammary gland which, together with the growth of the mammary gland, causes herniation of the areola and its exit through the point of least resistance, the thin skin of the nipple.
Grade I.- They present hypoplasia (incomplete development or lack of development) of the lower inner quadrants.
Lack of mammary gland and constriction affects the lower half of the breast.
The gland is herniated within the nipple-areola complex and there is a lack of volume in all four quadrants of the breast.
