A bit of anatomy first: to understand this part of your body.

The lip has its own structure, which allows us to work on each of its areas in different ways to achieve the desired effect. The most prominent anatomical landmarks in the lip area are the philtrum, the philtral columns, the Cupid’s bow, the vermilion, and its border.

In addition, although they are not part of the lip, we must take into account two folds: the nasolabial fold, which runs from the nasal wings to the corner of the mouth and borders the cheek; and the mentolabial fold, between the lower lip and the chin (also called the ‘marionette line’). The greater or lesser prominence of these folds will also impact the effect of our lip. We must not forget the vertical lines that form on the upper lip, known as the ‘barcode lines,’ which appear as part of the aging process in the area.

The oral commissure is located at the level of an imaginary line drawn from the inner edge of the iris. In the upper lip, the most striking feature is the Cupid’s bow, which lies between the two philtral columns and is the curve formed by the vermilion line between them.

How do the lips and the entire mouth area age?

With aging, we observe a deepening of the nasolabial and labiomental folds, which become more noticeable, marked, and pronounced. Over time, there is also an atrophy of lip volume and thinning of the upper lip, along with wrinkles on the upper lip commonly referred to as ‘barcode lines,’ and when they appear on the sides, we call them ‘marionette lines.

To evaluate the proportion of natural lips, several studies have been conducted and conclude that the perfect proportion is when the lower lip is twice as thick as the upper lip—that is, the upper lip occupies one-third of the mouth and the lower lip two-thirds. Another factor to consider when speaking about lips is not to forget the shape and volume of the chin, since, just like the facial folds, the chin also plays an important role. Therefore, beyond the more academic beauty standards, what really matters is seeking harmony and the beauty of our own lips.

“Not all lips are the same. Lips can have different volumes—thin or thick—different sizes, large or small, and also different shapes. Some lips are very straight, while others clearly define the anatomical structures we mentioned earlier. To achieve a natural result, it is important to consider the existing structure of the lip, which will determine the treatment to perform and how much volume can be added.

Procedure

The lip augmentation procedure is simple and can be performed with topical (cream) or local anesthesia. The method is carried out gradually, and you can immediately see the result, allowing you to decide if you want more volume in the area. A touch-up can be done after about 10 days.

Material used for lip filling

Currently, hyaluronic acid is the most commonly used lip filler. It is a very safe material, with extremely rare complications, and provides excellent results. We have different types of hyaluronic acid that allow us to work differently on lip contouring or volume. These hyaluronic acids have evolved, improving their characteristics compared to those used years ago, allowing lip volume while maintaining elasticity and natural expression, without it being obvious that a filler has been applied. Naturalness and harmony are the goals of our treatment.

Treatment of the cutaneous-mucosal line of the upper lip defines its contour and increases projection. We can also inject into the philtral columns to provide more definition in that area. To add volume, we work in the submucosal area of the vermilion, applying different techniques depending on the type of lip and the desired result. If vertical lines are present, they can also be treated with hyaluronic acid. We particularly like the blanching technique, which improves fine wrinkles without adding volume.
