What are hip dips?

Hip dips are these indentations that are marked in women at the level of the hips, especially between the iliac crest bone and the hip proper (anatomically called the greater trochanter).

The hip area is one of the elements that most define femininity, since the curve it forms is one of its characteristics. The straight line is associated with the idea of “masculine” and the curve with “feminine”.

How are hip dips treated?

As is always the case, what we really focus on is balance and compensation between areas, especially the waist. That is why we try to perfect this area by focusing on the aesthetic unity formed by the flanks (the lateral part of the waist) with the hips and thighs.

When there is an excess in the hips, the solution is liposuction. Evidently, it is not a matter of just emptying it, since we would empty it but without balance with the rest (we lose the shape). That is why we speak of liposculpture. What we want is to reduce volume while maintaining the feminine shape. We always treat the flanks and lower lumbar area together with the hips, as well as the lower area of the buttocks, in order to achieve a harmonic shape.

The video deals with the phenomenon known as “hip dips” or “hipties”, which are indentations that appear between the waist and hips of some women, giving them a shape that is sometimes compared to a guitar or a violin. This feature is completely natural and does not represent any pathology. However, some women may feel insecure or dissatisfied with their appearance because of these clefts. In terms of experience, this insecurity can affect their perception of themselves and their femininity. To improve the appearance of hip dips, certain exercises or surgical treatments, such as liposuction or fat injection, are recommended to make the area more homogeneous. In more extreme cases, hip augmentation implants are considered. It is essential for women to be aware that, regardless of their choice, the most important thing is how they feel about their body and their personal experience.

Treatments

Hip widening with fat or implants

More infrequently, we need hip augmentation or pelvic expansion surgery. This situation is found in women with accentuated hip dips, androgynous fat distribution (straight lines), patients with lipodystrophy (alteration of fat distribution due to medication) and in gender surgery (sex changes). If there is fat in other areas we can do a transplant, obtaining adipose tissue from the abdomen, flanks (so we also reduce the waist circumference) and grafting it in the hip area. But if there is no fat to model the hips we can put implants. This technique is our original and was first published in Aesthetic Plastic Surgery many years ago(Benito-Ruiz J et al.Hip and buttock implants to enhance the feminine contour for patients with HIV. Aesthetic Plast Surg. 2006 Jan-Feb;30(1):98-103). Initially we used it in patients without adipose tissue lost due to retroviral treatment and later we have also used it in patients who change gender or women who have not developed this area.

We have several possibilities depending on the projection (widening) we want to achieve. For a magnification of about 2 cm we can use a  oval cohesive silicone prosthesis under the tensor fascia latae muscle. The problem is that the space is very limited, so that we could only place a prosthesis of about 110 g -215 g and we could not achieve more than 2 cm of projection. The fascia lata is very powerful and not very extensible, so it limits tissue expansion.

We currently use a custom-designed prosthesis that can be placed on top of the muscle. The projection we get is about 3 or 4 cm. In addition, the patient’s own fat is associated (lipotransfer), to increase the thickness of the tissue and especially to smooth the contours of the implant. The disadvantage of this system is the cost of the implants.

The scar will be on the edge of the iliac crest, an area that is usually covered by underwear or a bikini.

Postoperative hip augmentation

After the operation to increase hips we must follow some recommendations. Since it is an area of great mobility we recommend not to sit for 7-10 days and avoid lower limb exercise for 6 weeks. The results are good being the biggest risk that the implant is displaced and need to be repositioned. That is why at the beginning it is necessary to be careful with sudden and intense movements.

The scar is hidden under the underwear.
