The study deals with the anatomy of the intermetacarpal cord artery and its branches and its involvement in the design of the dorsal metacarpal flap. The intermetacarpal artery originates from the deep palmar arch and supplies fingers. More specifically, it springs directly from the radial artery and gets divided into the arteries of the thumb and forefinger.

A flap is a vascularized tissue transfer to reconstruct a defect, and therefore every flap must have certain characteristics of size, function, circulation pattern, innervation, and so on. The metacarpal flap was described simultaneously by Maruyama and Quaba and there is controversy about whether to include the artery in the flap (Maruyama’s position) or not (Quaba). We have determined that it is not necessary to include the artery and Quaba design is therefore valid.

We chose the MBSRQ precisely because it is currently by far the most comprehensive and multidimensional metric tool to assess the body image. The psychometric validation of the Spanish version of the questionnaire has allowed us to reduce it to 45 items from the 69 items of the original version (which makes it easier for its administration).

The questionnaire is interesting for all areas of medicine associated with the body (oncologists, dermatologists, dieticians, reconstructive surgery, aesthetics, psychiatry, psychology, … and many more ). It allows to study scientifically the psychological effect of all that has to do with a variable that has so far been very subjective: body image.

In our particular case, along with the photos we take before and after the surgical procedure, it also gives us a valuable picture of what the patient really thinks of himself/herself.

The paper, written by Luis Botella García del Cid, Emma Ribas and Jesús Benito, has been published in the Revista Argentina de Clínica Psicológica, vol XVIII (November 2009).

On November 20, First National Congress of SETGRA (Spanish Society of fat transplanting applications) was held. I founded this society along with Dr. Serra Renom from Clínica Quirón (BCN) and Dr. Joan Fontdevila from the Hospital Clínico of Barcelona. With more than 100 surgeon from all over Spain attending, and apart from us, involving Dr. Di Fazzio and Dr. Andrea from Italy. He presented the latest applications of adipose tissue grafts in reconstructive and aesthetic surgery.

In my case, I presented the current state of “Stem Cells in the adipose tissue and Implementation of Adipose Tissue in Breast Augmentation.” The importance of adipose tissue, ie fat, which is where there is a greater number of stem cells and where it is easier to extract. These stem cells become endothelial cells and help the inserted tissue live. In addition, we use them in regenerative medicine research because with the right stimulus they can become bone cells, or neural, hepatic, myocardial, cartilage… In fact, stem cells from liposucted adipose tissue is being used for regeneration of myocardial infarction.

With some news or items related to reconstructive plastic surgery, some may wrongly conclude that by injecting stem cells, breast gets bigger. And this is not really so, this is a technique called Celution: we get fat by liposuction and divide it into two parts. With one part, we obtain the stem cells, and the other is processed by the Coleman system to purify the fat. The two parts are mixed and we get fat enriched with stem cells. The stem cell, as I noted above, tends to form blood vessels in adverse conditions and thereby improves blood circulation, thereby favoring the survival of fat grafts. It is this that increases the volume, not stem cells. Stem cells become endothelial cells, not mammary gland per se.

Of course, we strongly believe in the potential of adipose tissue not only as a filler but above all by the regenerative properties of stem cells. Deep vein thrombosis is popularly known outside the field of surgery as “economy class syndrome”. In plastic surgery is a complication which, although rare, is dangerous because there is always a risk of clots forming in the legs and in extreme cases these can lead to stroke and death.

The plastic surgery procedures with greatest risk are the abdominoplasty (with greatly increased risk if combined with another intraabdominal procedure), the belt lipectomy and TRAM flap breast reconstruction.

The Spanish Association of Aesthetic Plastic Surgery has published our work that addresses this issue for the first time in Spain with the aim of enhancing security and profilaxis during surgical interventions and where we present guidelines for the prevention of thromboembolic events tailored to Spanish plastic surgery.
