We went to Stockholm to attend the course “Beauty Through Science” for its sixth year, being one of the classics within the Plastic Surgery.
The first day was devoted exclusively to breast augmentation. The most remarkable session was the intervention of Patric Maxwell, USA, who presented a new simulation system called 4D. It is not yet on the market and I am afraid it will be very expensive when it hits the market, but it gives amazing results because with a simple picture with a special light, it gives you a perfect highlight of the breast and chest, and the outcome depending on the prosthesis you choose.
All teachers agreed that anatomical cohesive gel implants are the recommended choice as they provide a more natural and permanent result. The choice of implant should be made in terms of measures of the patients, discouraging larger volumes than the breast size as it increases the incidence of complications (mainly implant visibility and atrophy of the gland).
The second part of the morning was devoted to complications. The newest concept here is the possible role of biofilm in causing capsular contracture. The biofilm is the name that is given to a layer of bacteria that are “sleeping” on the surface of the implant and unresponsive to conventional antibiotic treatment. It is recommended to prevent it by irrigating the pocket and implant with an antiseptic solution or antibiotic (the latter solution may increase the organism resistance to antibiotics).
Just like Munich symposum, the role of acellular dermal matrix to solve coverage problems with implants was presented. This is not yet available in most European countries and it is quite expensive.
The second day was devoted to minimally invasive procedures (such as fillers and botulinum toxin). They also discussed the role of fat transplantation of stem cells derived from adipose tissue. Currently it is not entirely clear that enriching the adipose tissue with stem cells (Celution) increases the survival of transplanted fat. There is some evidence but no conclusive data. The proven advantage of these stem cells is that they become capillaries and improve the vascularization of tissue. The skin improves its appearance when the tissue is enriched with stem cells.
The course finished on the third day with facelift techniques. Nothing particularly new. The need to rejuvenate the area of the cheekbone by lifting the tissues or filling them with fat was stressed. It is the most important part of the facelift. Bruce Connell presented his experience over 50 years in performing facelifts and proved he is still a master, especially in the male lifting.
And all this while Stockholm was getting ready for the big wedding…the city was especially pretty. It is clean, wide and green…but with some weather quite not recommended for the Mediterraneans!
