End of last decade together with the introduction of new antirretroviral treatments for HIV infected patients , the first lipodistrophy cases started appearing . This syndrome is characterized by body fat distribution alterations in fat metabolization ( cholesterol and triglicerids ) and sugars .
These alterations create a distortioned body image which can create psychological problems and stopping retroviral treatments.
How do I know is a lipodistrophy?
The physical expression of a lipodistrophy can be a clear atrophy and thinness of specific body areas or fat tissue. The atrophy can affect face, buttocks , legs and arms and fat tissue affect basically back or abdomen.
Is it reversible?
We are unaware of all the aspects affecting lipodistrophy , but up to now it seems that is a chronic disease and we do not have effective treatments of its origin so that we have to find helping treatments to decrease the visible effects.
What Can I do if I have lipodistrophy ?
If you have HIV and you think your body might be changing and can be a lipodistrophy syndrome please ask your GP . He will evaluate if he has to make changes in your antiretroviral treatment and if he has to ad dan alternative therapy which will help stopping or reverting the process . If the patient doesn´t improve and you do not see any change or it worsens , you will have to visit an experienced plastic surgeon who will offer treatment alternatives .
Can I do anything to prevent lipodistrophy ?
If you are overweight it´s important to start a diet with your doctor to avoid fat tissue accumulation . As if you´re extremely thin you shoud start a diet to gain weight/ body mass to avoid first atrophia sympthoms. Strict diets can lead to accumulation or atrophy symptoms so it will be important to to know your previous medical history so your GP can supervise and control your treatment.
Once the lipodistrophy is diagnosed by your surgeon , what it can be done to prevent worsening ?
A diet according to your needs , exercise , psichological support and medical control are really remarkable factors to control lipodistrophy . Your doctor can make you other suggestions right for you , as supplements for instance .
Is it true that some hormones can help improve lipodistrophy ?
Some hormones as the grow hormone and tetosterone have beeing used to treat distrophy but its long term potential second effects and complications do not allow them to become a generalized treatment.
Which treatments are available?
There are many options for each specific case but , up to now fat tissue injections is the best choice.
Why body fat is used to treat atrophies instead of a sintetic material to avoid needing it?
If patient´s body fat is used , we avoid the possibility of rejection or complications associated to material composition .Sintetic materials present very few complications but in this aspect , fat inocuity succed.
Why fat injection is not a generalized treatment ?
Some specialists do not like autologous purified adipose graft treatment obtained from patient´s fat since is boeing reabsorbed , goes away or in a meaningful number of cases , the patient does not have enough body fat to follow the treatment. If lipodistrophy patients loose facial fat why they do not loose injected fat ?
Autologous purified adipose grafts are obtained from body areas lipodistrophy resistant in where there are some accumulations sometimes . Lipodistrophy affects an specific kind of fat but does not affect a particular body area , if we inject another kind of fat to that area which is resistant to the treatment we do not necessarily have to loose it, even if the lipodistrophy is quite advanced in its process.
Is body fat the best future option?
Absolutely, take into account that once one the purified autologous fat grafts are injected , the skin reacts as if nothing had happened . If , in the future we have to cut it , trest a cist or treat a skin cancer we won´t have any problem ( but we World have problems with sintetic implants ). Moreover, we will always have the option of using sintetic implants in the future if necessary.

HOla, soy una chica joven……que no tengo Sida…pero por unas diarreas,…he perdido en dos meses….grasa facial y musculo, craneal,…..y detras detras…del cuello……es un TRauma muy grande verme al espejo cada dia…….porque mi peso ahora es de 63 kg,…….pero mi cara……esta orrible…..!!!!!!!!Me gustaria una posible respuesta……..Gracias……

A falta de una visita en la consulta para definir la mejor solución, si hay alguna zona donante el mejor tratamiento es la inyección de tejido adiposo, dado que es la mejor manera de reconstituir el tejido perdido. Si no existe grasa puede usarse material sintético como el Radiesse.

En su caso la mejor opción y la que mejor resultado da en nuestras manos es transferir grasa desde alguna otra localización a la cara. Habría que verle en la consulta para determinar lo que hay que hacer y enseñarle ejemplos. Si no tiene grasa para transferir habría que utilizar algún material como Sculptra o Radiesse.

Pero, para realizar los microinjertos de tejido adiposo autólogo purificado, es necesario intervención quirúrgica???
O, simplemente consiste en realizarme pinchazos de forma ambulatoria. Sin anestesia ni quirófano.

Es una intervención quirúrgica en la que hay que obtener los injertos y luego trasplantarlos a la cara. Se necesita anestesia local o local y sedación en función del volumen a implantar.
