Dupuytren’s Contracture

This disease affects the palm of the hands. Also it can happen at the feet (Lederhose’s disease) and at the penis (Peyronie). It is more frequent in men, especially manual workers and it usually has a family inheritance. It affects more in the fingers annular and little finger and more seldom in thumb.

It consists of a thickening of the palmar fascia. The palmar fascia is a fibrous structure that is exactly under and in intimate contact with the skin, and it covers the deep structures of the hand (tendons, vessels and nerves). First of all some nodules turn up in the palm of the hand. These nodules are fixed to the skin and keep on joining until they form a band, which seems a very superficial tendon. This band shortens and ends up deforming the fingers, which are bent. The severity of the condition is in the fingers depending on the degree of flexion that it causes.

The treatment is surgical and is indicated when the articulations start to bend and the patient can completely not pull the finger. It consists to removing the ill fascia (fasciectomy). The operation is more difficult how more flexioned the finger is and more time the disease is present. It does not require stay in the hospital. The results are good in most of the cases.

After taking off the stitches a very intense physiotherapy has to start to win all the movement of flexoextension. In very severe cases it can be that the interphalangeal joint does not achieve the whole range of movement again because of the affectation of the volar plate or the shortening of the flexor tendons.

Buenas tardes
es la tercera vez que me van a operar de un problema en el pie llamado LEDERHOSE es repetitivo en cuadro 2007, 2008 y ahora 2010.
No soy diabetica, no fumo no tomo alcohol, quisiera saber a que se debe. Tengo 55 años y mi sobrino de 33 tuvo lo mismo a el lo operaron 2 veces. Espero vuestros comentarios. Muchas gracias.

  Muchas gracias a ti por visitar nuestra web y dejarnos un comentario. Efectivamente, Lederhose es la forma en el pie de la enfermedad de Dupuytren. Además de las causas que menciona, hay un componente genético evidente. No se conoce la causa y desgraciadamente puede volver a aparecer tras la cirugía.

Hola,estoy operada por tres veces de la mano derecha,una de la izquierda y hace 15 dias de la planta del pie.No soy fumadora ni tomo alcohol, tampoco antecedentes familiares.He leido del Nuevo tratamiento con colagenasa.Puede informarme?Debo tratarme de Nuevo una mano.

  No tenemos experiencia en el uso de colagenasa. Lo que hacemos nosotros es romper el tejido cicatricial e inyectar tejido adiposo. Éste es un tratamiento descrito recientemente pero que parece reducir las recidivas a largo plazo. Para el tratamiento de colagenasa le recomiendo que contacte con el Dr. Angel Ferreres del Institut Kaplan de Barcelona,

  La solución es quirúrgica. Primero se opera una mano y cuando esté recuperado, la otra. Tendría que verle en la consulta para determinar lo que hay que hacer, en cualquier caso. Si le interesa puede llamarnos al 93 252 23 49.
